What is photodynamic therapy?
Photodynamic therapy is a treatment in which a solution is applied to the skin and then activated by light.  It is FDA approved for treating pre-cancers, though is used as an “off label” application to treat acne, enlarged oil glands, and rejuvenate the skin.
 
What should I expect?
First, we will have you wash your face with a cleanser.   Next, the excess oils are removed using rubbing alcohol or a light chemical peel.  The photodynamic solution (called Levulan) is then applied and you may feel a mild stinging.   Depending on the condition being treated, the solution will be left on the skin for a minimum of 30 minutes. During this time, you may leave the office though it is important to keep all visible light off your skin, as this will activate the solution prematurely.  This includes avoiding being exposed to sunlight passing through glass while driving.   Because the solution is activated by visible light (and not ultraviolet light), sunscreen is not effective.  Hence, you will need to bring a wide brim hat and protective clothing (such as a scarf, bandanna, or hooded jacket) with you to your appointment.    Upon returning to the office, the solution will be activated with a blue light or other controlled light source.  During this time, some stinging may occur.  A fan is helpful for alleviating this sensation.  The type of response that follows is variable.   In most patients, the skin will be pink and/or scaly for 2-4 days; rarely, patients become swollen and blistered.

How many treatments will I need?
For acne and enlarged oil glands, 3-5 treatments are needed and should be spaced apart every 3-4 weeks. Spacing them further apart may diminish some of the benefit.  For skin rejuvenation and pre cancers, 2-4 treatments are need and can be spaced as far apart as desired (though no sooner than every 3 weeks).

How do I prepare for the treatments?
1.    Avoid using any irritating skin care products (including prescriptions such as Retin A, Renova, Tazorac, or Differin) for 3 days prior to the treatment.
2.    BE SURE to bring a large brimmed HAT and scarf or bandanna to your treatment- otherwise, we will need to reschedule your treatment for your own safety.
    
What will my post treatment instructions be?
1.    Keep the treated area protected from sunlight for 48 hours.    
     •    For faces, wear a wide brimmed hat and scarf when outdoors x 48 hours.
     •    For hands, wear gloves when outdoors x 48 hours
     •    For the neck and chest treatments, wear a turtleneck when outdoors x 48 hours.
2.    Indoor lighting is okay as long as it is 3 feet away from the treated skin.
3.    Sunscreen is NOT effective protection for the 48 hours post-treatment period.
4.    If peeling occurs, apply moisturizers to the sites as needed.  
5.    It is okay to be exposed to sunlight 48 hours after the procedure; however, to optimize healthy skin, wear sunscreen daily.
